As the mother of an adult child with significant special needs, I noticed that there was very limited information as to what happens when your child ages out of the school system or turns 18. In the special needs world, it's called "falling off a cliff" or "graduating to the couch."
The biggest question is: What's next?!
My purpose is to help parents navigate this information.

What is a Transition meeting?
A typical Transition meeting can last from 1-2 hours. We will cover a variety of topics and "to-dos" that come up as your child enters into adulthood. I will give you a check list of tasks to help you get started on getting adults supports & services in place.
Some of the topics we cover: Guardianship/conservatorship options, SSI, waivers, Special Needs Trusts, SNAP, camp, Family Support Funds, banking/financial steps and many more.
