helping parents navigate the transition to adult services and support
TRANSITIONS
{noun} the process or a period of changing from one state or condition to another.
As the mother of an adult child with significant special needs, I noticed that there was very limited information as to what happens when your child ages out of the school system or turns 18. In the special needs world, it's called "falling off a cliff" or "graduating to the couch."
The biggest question is: What's next?!
My purpose is to help parents navigate this information.

Some of the topics we cover: Guardianship/conservatorship options, SSI, waivers, Special Needs Trusts, SNAP, camp, Family Support Funds, banking/financial steps and many more.
