Helping parents navigate the transition from child to adult services and support

TRANSITIONS
{noun} the process or a period of changing from one state or condition to another.
As the mother of an adult child with significant special needs, I noticed that there was very limited information as to what happens when your child ages out of the school system. In the special needs world, it's called "falling off a cliff."
The biggest question is: What now?!
My purpose is to help parents navigate this information.
A Transition Meeting can help you work through the "to-do" list of things to prepare yourself and your child for the transition to adult support & services.
